What Is Hyperpigmentation?

Hyperpigmentation refers to patches of skin that become darker than surrounding areas. It occurs when there is an excess production of melanin (the pigment that gives skin its color). Hyperpigmentation can look like flat or raised patches of skin. The darkened areas may be limited to small regions or cover larger body areas. While hyperpigmentation is usually harmless, many people seek treatment for cosmetic reasons to address their uneven skin tone. This condition can affect individuals with all skin colors and types. In rare cases, hyperpigmentation may
be an early sign of skin cancer, so it is essential to have regular checks.

Types of Hyperpigmentation

Hyperpigmentation can appear in different forms, depending on the cause and the patient’s anatomy, genetics, and sun exposure. Here are the main skin concerns that people typically have:

Melasma

Melasma causes light brown or tan patches on the face. It is triggered by sun exposure, hormones, or pregnancy.
Sunspots are flat, tan, or brown formations caused by sun exposure. They typically appear on the face, hands, and arms in middle age.
PIH causes dark spots to appear where previous acne scarring has healed. The patches occur when excess melanin is produced during the recovery process. Hyperpigmentation may appear after mild acne or severe acne scars.

These are tan, brown, or black spots that form on exposed areas due to solar damage. They are associated with aging and typically appear on the face, hands, arms, and other sites.

Cafe-au-lait spots are flat, light brown birthmarks that vary in size. They are caused by a localized increase of melanin.

These are small, flat, tan spots that mainly appear on sun-exposed skin. They are hereditary and associated with fair skin types.

What Causes Uneven Skin Tone?

Sun Exposure

Prolonged sun exposure can lead to hyperpigmentation. It occurs because UV rays from the sun stimulate melanin, causing it to produce more pigment. Areas of skin that have had more exposure, like the face, arms, and hands, are more prone to developing hyperpigmentation. Using sunscreen and protective clothing helps to prevent this condition.
Fluctuations in hormones like estrogen and progesterone can stimulate melanin production and cause areas of skin to become darker. This type of hyperpigmentation often appears as patches on the face and is sometimes called melasma if it occurs in pregnant women. Keeping hormones balanced may help to prevent and treat this condition.
As skin ages, it becomes more prone to hyperpigmentation since cell turnover decreases. Age and sun spots are common problems in older adults, even with limited sun exposure. Genetics can also play a role in the development of hyperpigmentation, especially in people with darker tones. Those with a family history of skin discoloration tend to be more susceptible. While the natural aging process and genetic traits cannot be controlled, diligent sun protection and avoiding irritants may help mitigate pigment changes from these causes.
Laser hair removal uses light energy to target melanin in hair follicles. This process can also affect melanin in the surrounding skin, which may cause hyperpigmentation. Proper treatment settings and care are required during laser hair removal to minimize the risk of this side effect. However, it is not always possible to completely prevent it.

How Is Hyperpigmentation Treated?

We treat hyperpigmentation with an Aerolase® laser by directing focused beams of light at the affected areas of the skin. This device targets and destroys overproduced melanin pigment in the skin. Multiple treatments spaced weeks apart are typically needed to achieve the desired results. Each Aerolase® session lasts around 10 to 20 minutes on average. The laser gradually breaks down the excess melanin during these treatments, making the skin tone more even. This procedure is often combined with topical medications to achieve the results faster.

Preparation for Hyperpigmentation Treatment

Our laser treatment requires some preparation before it can be provided. If the patient has a medical condition affecting the skin or immune system, they should consult their primary care physician to ensure this intervention is safe. Here are the main steps in preparation for laser procedures:

Consultation

The patient meets with our doctor to discuss their skin concerns and goals. The provider evaluates the skin and determines if Aerolase® is an appropriate option. A treatment plan is also created to address all issues.

The doctor reviews medical history, including any conditions or medications that can cause complications. The patient may be asked to stop taking certain pharmaceuticals or supplements 1-2 weeks before treatment.
The patient reviews and signs consent forms indicating that they understand the potential risks and benefits of Aerolase® laser therapy. This includes possible side effects, such as redness and swelling.
Patients should avoid unprotected sun exposure for 2 weeks before and after treatments, or longer if possible.
To enhance the results, our doctor may recommend using retinoids, alpha hydroxy acids, or hydroquinone four weeks prior to the procedure.

Hyperpigmentation Treatment Procedure

The Aerolase® device does not cause pain or damage the skin, so anesthesia
is unnecessary. The doctor thoroughly cleans the treatment zone and applies
a cold gel to protect the skin’s surface. The Aerolase® laser handpiece is then moved over the target area systematically. It penetrates deep into the skin to target melanin and disperse patches of hyperpigmentation. Patients feel a warm sensation as the laser pulses. Treatment time depends on the size of the target area. Most small patches can be covered in 5-20 minutes. After the procedure, the doctor removes the gel from the patient’s skin.

Recovery After Laser Treatments

The recovery period after a hyperpigmentation treatment session with Aerolase® is typically short and involves no serious symptoms. Most people do not require any downtime. There is little to no pain, swelling, redness, or irritation associated with this laser treatment. Patients can usually return to their normal activities immediately after the procedure.
Over the next few days, the treated areas gradually get darker as the melanin comes to the surface before it sheds off. Patients notice a lightening of the pigmented areas within a week or two. Multiple sessions spaced several weeks apart may be necessary to see optimal results. The non-invasive nature of the Aerolase® technology makes this a very tolerable treatment.
Strict sun protection is essential during recovery to protect and maintain the improvements achieved. Overall, Aerolase® laser offers a low-risk and convenient option for treating stubborn hyperpigmentation and making the skin tone more even.
